Individuals new to our Center will be asked to submit paperwork prior to scheduling an appointment. You may access all necessary forms via the forms link or we will mail them to you. Return completed forms by fax to (336) 334-4475 or mail them to 300 Ferguson Building, UNCG, Greensboro, NC 27402.
For information about fees and billing please call (336) 334-5939 and ask for Mrs. Olga Olejarczyk. The Center is a Medicare provider for hearing services only. Payment methods include cash, personal checks, Mastercard or Visa.
Adverse Weather and Emergency Closings Policy: If the university is closed due to adverse weather or emergency evacuation the Center will not open for services. If there is a delay in opening the university the Center will follow the same schedule of opening. If you have questions about whether or not your appointment stands please call the office at 336-334-5939.
